Boston Scientific Corporation BSX lowered its 2026 sales and earnings outlook after slower WATCHMAN demand, U.S. electrophysiology share losses and limited operating leverage weakened near-term visibility.
The cut shifts the investment case from durable portfolio growth toward execution. Investors now must judge whether current pressure is a transition that clears by 2028 or the start of a longer stretch of slower growth and constrained margins.
BSX Cuts Growth and Earnings Expectations
Boston Scientific now expects reported sales growth of 5.5-6.5%, below its prior 7-8.5% range. Organic sales growth guidance fell to 5-6% from 6.5-8%, while adjusted earnings expectations declined to $3.28-$3.32 per share from $3.34-$3.41.
The third-quarter outlook also points to a sharper slowdown. Management expects reported and organic sales growth of 3-5% and adjusted earnings of 80-82 cents per share. The Zacks Consensus Estimate for third-quarter earnings stands at 84 cents, leaving the company’s range below the consensus mark.

Boston Scientific’s Product Mix Pressures Margins
Lower WATCHMAN and electrophysiology sales are expected to weaken product mix and push full-year adjusted gross margin below the 2025 level. Boston Scientific also reduced expected adjusted operating margin expansion to 0-25 basis points from its prior 50-75 basis-point range.
Supply-chain and quality-system investments remain elevated, limiting the conversion of revenue growth into earnings leverage. This matters because the company is still expanding across much of its portfolio, yet weaker mix in high-value cardiovascular franchises may keep margin progress muted.
BSX Pushes Key Recovery Catalysts Into 2027
Competitive gaps will not close quickly. FARAWAVE Ultra and intracardiac echocardiography products are expected in the second half of 2027, while the PRECEDENT defibrillator platform is also not expected until that period. Management does not anticipate an electrophysiology recovery before those launches.
That timing leaves Boston Scientific exposed while rivals continue to advance. Abbott Laboratories ABT entered 2026 with new U.S. approvals in electrophysiology and later reaffirmed 6.5-7.5% comparable sales growth guidance. Zimmer Biomet Holdings, Inc. ZBH, another medical-device peer, reported 2.9% organic constant-currency growth in the first quarter and updated its 2026 outlook.
Boston Scientific’s Restructuring Delays the Payoff
The restructuring program targets about $500 million in run-rate savings exiting 2029. More than half of the savings and related program spending are expected by the end of 2027, so the near-term period carries both implementation costs and only partial benefits.
Management expects revenue growth below its weighted average market growth rate and limited adjusted earnings growth in 2027. A broader recovery is therefore pushed toward 2028, when product launches, cost savings and a more stable mix may begin to work together.
Meanwhile, shares of Boston Scientific have gained 3.7% since the second-quarter earnings announcement, against the industry’s 1.5% decline. This is primarily attributable to the second-quarter earnings beat and continued growth across much of Boston Scientific’s portfolio despite the reduced outlook.

Why BSX’s Current Signals Favor Caution
The bottom line is that Boston Scientific still has a broad franchise, international growth and pipeline depth, but its reduced outlook makes the path to recovery longer and less visible. WATCHMAN weakness, electrophysiology competition and delayed margin leverage now outweigh the near-term offsets.
BSX currently carries a Zacks Rank #5 (Strong Sell), which aligns with downward earnings estimate revisions and weaker visibility. Its Value Style Score of C, Growth Score of C, Momentum Score of C and VGM Score of C show no clear style-based strength to offset the unfavorable rank. The combination supports a balanced but cautious reading rather than a firm conclusion about the stock’s longer-term outcome.
