Gilead Sciences GILD announced the FDA approval of Bixlenvo (bictegravir 75 mg/lenacapavir 50 mg), a once-daily single-tablet regimen for virologically suppressed adults with HIV, including those taking complex treatment regimens and unable to use currently available single-tablet options.
Bixlenvo combines bictegravir, a guideline-recommended integrase strand transfer inhibitor with a high barrier to resistance, and lenacapavir, a first-in-class capsid inhibitor with a novel mechanism of action and no cross-resistance to existing antiretrovirals. The approval makes Bixlenvo the smallest single-tablet regimen available and the first and only such option specifically addressing virologically suppressed patients on complex regimens.
Gilead’s lenacapavir is marketed under two brand names, Yeztugo and Sunlenca. Yeztugo is approved for pre-exposure prophylaxis, while Sunlenca is approved for treating HIV-1 infection in heavily treatment-experienced adults.
The FDA nod for Bixlenvo marks a notable milestone for Gilead's HIV franchise as it broadens the company's single-tablet treatment portfolio to a population whose needs may not be adequately addressed by existing therapies. Bixlenvo provides a once-daily oral option that combines two agents targeting different stages of the HIV life cycle, expanding treatment choices for patients who cannot use existing single-tablet regimens. However, there is currently no cure for HIV or AIDS.
Year to date, GILD shares have gained 21.3% compared with the industry's growth of 11.8%.

Rationale Behind FDA Nod for Gilead’s Bixlenvo in HIV
The FDA’s decision was supported by results from Gilead’s phase III ARTISTRY-1 and ARTISTRY-2 studies of Bixlenvo for HIV. ARTISTRY-1 evaluated virologically suppressed adults switching from complex multi-tablet regimens, while ARTISTRY-2 assessed patients switching from Biktarvy. Both studies demonstrated non-inferior efficacy in maintaining virologic suppression through Week 48, supporting the regimen's ability to preserve viral control after treatment simplification.
The phase III ARTISTRY-1 study was particularly relevant as it closely reflected the patient population Bixlenvo is intended to treat. Participants had a median age of 60 years and extensive HIV treatment histories, with a median treatment duration of 28 years. At the start of the study, the patients were taking between two and 11 pills a day, and around 40% were taking antiretroviral therapy more than once daily. In addition, 67% had resistance to nucleoside reverse transcriptase inhibitors, while 81% were on complex treatment regimens due to antiretroviral resistance. Findings from the study highlighted Bixlenvo’s potential value for patients with long and complex treatment histories.
GILD’s Bixlenvo was also generally well tolerated across the ARTISTRY studies, with no new or significant safety concerns. The most common side effects, reported in at least 2% of participants, were headache, nausea and diarrhea. In ARTISTRY-1, patients who switched from complex multi-tablet regimens also showed improvements in certain fasting lipid measures and treatment satisfaction. Meanwhile, ARTISTRY-2 found that switching to Bixlenvo did not significantly affect body weight.
Bixlenvo's dosing profile further strengthens its value as a treatment-simplification option. Patients require a two-day initiation dose with Sunlenca, followed thereafter by once-daily oral Bixlenvo. Its distinct mechanism of action expands Gilead's HIV treatment offering while addressing an important gap for virologically suppressed adults who cannot readily transition to existing single-tablet therapies.
